What Imodium is
- Replace lost fluid first: Aspin’s leaflet says that in diarrhoea, especially in children, fluid and electrolyte replacement is the most important measure. The UK patient leaflet says to ask your pharmacist about rehydration therapy to replace lost salts.
- Prescription status is not settled: Aspin’s leaflet gives no condition of sale, in English or Urdu; its only instruction is to use it as the doctor directs. Aspin’s product page says “To be sold only on prescription.”, in a disclaimer it also shows on other products. Until the carton settles it, we ask for a prescription.
- DRAP controls Imodium’s price: its price index sets Rs 397.83 for a pack of 60 capsules, effective 23 July 2026. We confirm the price of a strip on WhatsApp.
What is in it, with DRAP registration
From DRAP’s price index and Aspin’s leaflet; DRAP’s registry words the label claim differently (below).
| Product | Ingredient | Amount |
|---|---|---|
| Imodium 2mg capsule (DRAP reg. 006159) | Ingredient: Loperamide hydrochloride | Amount: 2 mg per capsule |
Aspin’s leaflet says the pack is 5 strips of 12 capsules, and describes the capsules as having a green cap and an opaque dark grey body.
DRAP’s registry label claim reads “Loperamide.......2mg”; DRAP’s price index and Aspin’s leaflet give the 2 mg as loperamide hydrochloride.
DRAP’s price index lists no other Imodium product.
What it is used for
Pakistan’s leaflet and the UK label list different uses, so both are shown.
| Source | Uses |
|---|---|
| Pakistan, Aspin (Imodium) | Symptomatic control of acute and chronic diarrhoea; in people with an ileostomy, to reduce the number and volume of stools and harden them. |
| UK, Imodium 2mg capsules | Acute diarrhoea in adults and children aged 12 and over; acute episodes of diarrhoea associated with irritable bowel syndrome in adults aged 18 and over, after a doctor’s diagnosis. |
How to take it
The dose differs for acute and chronic diarrhoea, and for adults and children, according to Aspin Pharma’s Pakistani leaflet for Imodium, so the choice belongs to your prescriber. Take it as they direct.
- Maximum: Aspin’s leaflet says the maximum for acute and chronic diarrhoea is 8 capsules (16 mg) a day for adults; for children it must be related to body weight and should not exceed 8 capsules a day. The UK product information says the maximum daily dose should not exceed 6 capsules (12 mg).
- Aspin’s leaflet says the capsules should be taken with liquid.
- How long: Aspin’s leaflet says that in acute diarrhoea, if there is no improvement within 48 hours, it should be stopped and you should see a doctor.
- Storage: Aspin’s leaflet says do not store above 30°C, and protect from light and moisture.
Side effects
Constipation, wind, nausea and headache were the most common in clinical trials, according to Aspin Pharma’s Pakistani leaflet for Imodium.
| Source | Side effects |
|---|---|
| Pakistan, Aspin (most common) | Constipation; flatulence; nausea; headache; in chronic diarrhoea, also dizziness. |
| Pakistan, Aspin (very rare) | Severe allergic reactions and severe skin reactions; blocked or paralysed bowel (ileus) and megacolon; loss of consciousness; urinary retention. |
| UK product information | Also lists acute pancreatitis, which the Pakistani leaflet does not. |
Driving: Aspin’s leaflet says tiredness, dizziness or drowsiness may occur with diarrhoea treated with loperamide, so take care when driving or using machinery.
Who should not take it, or should ask a doctor first
Pakistan’s leaflet and the UK label word these differently; each line names its source.
| Who | What the sources say |
|---|---|
| Children | Pakistan: not under 2 years; ages 2 to 6 only with a medical prescription and supervision. UK: contraindicated under 12 years. |
| Blood in the stools with high fever, ulcerative colitis, bacterial gut infection, or colitis after antibiotics | Pakistan: should not be used as the primary therapy. UK: contraindicated. |
| When the gut must not be slowed | Pakistan: should not be used, because of the risk of ileus, megacolon and toxic megacolon. |
| Liver disease | Pakistan: use with caution, and watch for signs of nervous-system toxicity. |
| People with AIDS | Pakistan: stop at the earliest signs of abdominal distension. |
Aspin’s leaflet says loperamide has been abused as an opioid substitute by people with opioid addiction.
Other medicines
These medicines raise loperamide or are raised by it, according to Aspin Pharma’s Pakistani leaflet for Imodium and the UK product information.
| Medicine | What the sources say |
|---|---|
| Quinidine, ritonavir | Raised loperamide levels two to three times. |
| Itraconazole, ketoconazole, gemfibrozil | Raised loperamide levels; itraconazole with gemfibrozil raised total exposure about 13 times. |
| Desmopressin taken by mouth | Loperamide raised desmopressin levels about three times. |
Pregnancy and breastfeeding
Ask your doctor before taking it if you are pregnant or breastfeeding.
| Who | What the sources say |
|---|---|
| Pregnancy | Pakistan: the benefits should be weighed against potential hazards, especially in the first three months. UK: not advisable in pregnancy, especially the first three months. |
| Breastfeeding | Pakistan: not recommended, because small amounts may appear in breast milk. UK: not recommended during breastfeeding. NHS: it’s OK to take loperamide while breastfeeding. |

DRAP registration
- IMODIUM CAPSULES (006159), Aspin Pharma (Pvt.) Ltd. DRAP’s registry shows: Reviewed, “Not Verified”.
DRAP publishes no definition of this verification flag, so we can’t tell you what it means. We searched DRAP’s website for “Imodium” and “loperamide” on 30 October 2026 and found no alert naming Imodium or loperamide. When we tested it, the search did not find a word that appears only inside an alert’s attached PDF, so an alert that names a product only in its PDF may not show up. DRAP describes its registered products index as a provisional list that “cannot be used as a reference for any purpose”. Checked against the registry on .
