What Flagyl is

Flagyl (metronidazole, 400 mg tablets or 200 mg/5 ml suspension) is an antibiotic and anti-amoebic medicine for infections caused by anaerobic bacteria, and for parasitic infections caused by Trichomonas vaginalis or Entamoeba histolytica, according to Hoechst Pakistan’s own product page for Flagyl. We dispense it on prescription.

  • Flagyl 400mg tablet is DRAP reg. 000827 (DRAP registry entry) and the suspension is DRAP reg. 001214.
  • Made by Hoechst Pakistan Limited in Karachi: the company’s own financial statements say it was formerly sanofi-aventis Pakistan Limited and changed its name in September 2023.
  • Finish the course: the NHS says carry on taking metronidazole until you have finished the course, even if you feel better, because if you stop early the infection could come back.
  • We found no Pakistani leaflet for Flagyl online, so no Pakistani document we could find states its condition of sale or how to take it. Dispensing it on prescription is our own policy, and the clinical facts on this page come from the sources named where they are used.
  • DRAP controls Flagyl’s price: its price index sets Rs 999.45 for 200 tablets of 400 mg (20 strips of 10) and Rs 162.67 for a 120 ml bottle of suspension, effective 27 July 2026, and we charge less.

What is in each form

One active ingredient, metronidazole, from the label claims in DRAP’s registry.

Active ingredient in each Flagyl form, with DRAP registration
FormIngredientAmount
Flagyl 400mg tablet (DRAP reg. 000827)Ingredient: MetronidazoleAmount: 400 mg per tablet
Flagyl suspension (DRAP reg. 001214)Ingredient: Benzoyl metronidazole 320 mg, equal to metronidazoleAmount: 200 mg per 5 ml

The suspension contains metronidazole as benzoyl metronidazole, the form used in liquids; DRAP’s record gives 320 mg of it in each 5 ml, equal to 200 mg of metronidazole.

What it is used for

Infections caused by bacteria that live without oxygen (anaerobes), and certain parasites.

Uses of metronidazole, by source
SourceUses
Pakistan, Hoechst (Flagyl)Preventing and treating infections in which anaerobic bacteria have been identified or are suspected; parasitic infections caused by Trichomonas vaginalis or Entamoeba histolytica, which causes diarrhoeal disease.
UK, metronidazole 400 mgPreventing infection after surgery; anaerobic infections such as septicaemia; trichomoniasis; bacterial vaginosis; all forms of amoebiasis; giardiasis; acute ulcerative gingivitis; infected leg ulcers and pressure sores; and acute dental infections.

How to take it

The dose and how long you take it depend on the type of infection and how serious it is, according to the NHS, so the choice belongs to your prescriber. Take it as they direct.

  • Tablets: the UK label says take them during or after a meal, and the NHS says swallow them whole with a drink of water.
  • Suspension: the sources differ. The South African label for Flagyl suspension (sanofi-aventis) says it is recommended to take it at least 1 hour before food, while the NHS says metronidazole liquid does not need to be taken with or after food. Follow your doctor or our pharmacist, and shake the bottle before each dose.
  • Longer courses: the UK label says that if treatment for more than 10 days is needed, regular check-ups and blood tests are advised, watching for nerve problems such as tingling, unsteadiness, dizziness and fits.

Who should not take it, or should ask a doctor first

From the UK product information for metronidazole 400 mg tablets unless another source is named.

  • Must not take it: anyone allergic to metronidazole or other nitroimidazole medicines.
  • Cockayne syndrome: the UK label says severe liver damage, sometimes fatal and very rapid, has been reported in people with this rare genetic condition, so metronidazole should not be used unless the benefit outweighs the risk and there is no alternative.
  • Ask first: the NHS says tell your doctor if you have liver problems or a disease of the nervous system, or are having dialysis.

Other medicines

Besides alcohol, several medicines interact with metronidazole.

The UK label and the NHS word this differently, so both are shown. Ask your doctor first.

Side effects

A metallic taste is a well-known one: the NHS says metronidazole may give you a slight metallic taste in your mouth.

NHSA metallic taste in your mouth or a furry tongue; feeling or being sick; diarrhoea.
UK labelDarker urine, caused by the medicine itself; patients should be warned of it.

Driving: the UK label says patients should be warned about possible drowsiness, dizziness, confusion, hallucinations, fits or brief vision problems, and not drive or use machinery if these occur.

Flagyl Plus and Flagyl injection: different products

Flagyl Plus is a different medicine: DRAP’s records give it metronidazole with a second ingredient, diloxanide furoate, as tablets (DRAP reg. 055977) and as a suspension (DRAP reg. 064850).

Flagyl Plus suspension’s strength is given per 10 ml, while plain Flagyl suspension is 200 mg per 5 ml, so one can’t be swapped for the other. We don’t sell Flagyl Plus on this page.

Flagyl is also registered as a 500 mg in 100 ml infusion (DRAP registry entry), given into a vein. We don’t sell it online.

Checking your pack is genuine

DRAP has issued three alerts about batches of Flagyl injection (infusion), and none about Flagyl tablets or suspension, as of 28 October 2026.

  • Check that the batch number on the carton matches the one on the strip or bottle, and that the pack is sealed.
  • The suspension is registered with DRAP under number 001214 as benzoyl metronidazole, and the tablet under 000827. If a pack looks different from your last one, ask our pharmacist before taking it.

DRAP recall alert (provincial government), Flagyl infusion, 4 March 2025: Flagyl infusion (metronidazole 500 mg), batch AD460, declared substandard because of visible particles.

DRAP Alert No. I/S/01-25-13, 28 January 2025: Flagyl injection, batch AD482, declared substandard on the endotoxin test.

Returns and replacements

Medicines and supplements can’t be returned once they have left our pharmacy.

  • Returned medicine can never go back into saleable stock. That is a safety rule, not a commercial one.
  • If an item arrives damaged or wrong, tell us on WhatsApp within 24 hours and we replace it. That is us fixing our mistake, not a return.
  • Anything that arrives expired or within a month of expiry we replace or refund, your choice, with no time limit.

Frequently asked questions

What is Flagyl 400mg used for?

Flagyl is metronidazole. Hoechst Pakistan’s own product page says it treats and prevents infections caused by anaerobic bacteria, and parasitic infections caused by Trichomonas vaginalis or Entamoeba histolytica, which causes diarrhoeal disease.

Can I drink alcohol with Flagyl?

No. The UK product information says no alcohol during treatment and for at least 48 hours afterwards, and the US label for Flagyl says at least three days afterwards. We suggest waiting 3 days after your last dose.

Why does Flagyl leave a metallic taste?

A metallic taste is a known side effect: the NHS says metronidazole may give you a slight metallic taste in your mouth.

Is Flagyl Plus the same as Flagyl?

No. DRAP’s records show Flagyl Plus contains diloxanide furoate as well as metronidazole. It is a different product with its own registration.

Do you need a prescription to buy Flagyl?

We dispense Flagyl on prescription. We found no Pakistani leaflet online that states its condition of sale, so this is our own policy.

DRAP publishes no definition of this verification flag, so we can’t tell you what it means. We searched DRAP’s website for “Flagyl” and “metronidazole” on 28 October 2026 and found three alerts naming batches of Flagyl injection, and none naming Flagyl tablets or suspension (below). When we tested it, the search did not find a word that appears only inside an alert’s attached PDF, so an alert that names a product only in its PDF may not show up. DRAP describes its registered products index as a provisional list that “cannot be used as a reference for any purpose”. Checked against the registry on .
